De Regenboog Groep is committed to helping people with problematic debts, homeless people, people with addictions, undocumented people, people with psychiatric problems and people who cannot find their way out of problems on their own.
In the "Accompanied Home" team, we assist people on an outpatient basis in their homes. The team gets behind the front door, mediates with care agencies and helps them get a grip on life again. Above all, the social workers are characterized by a person-oriented approach. We go the extra mile to make a difference and do whatever it takes to help someone move on. In doing so, we also support our colleagues at different locations and can move quickly when the situation calls for it.
Specifically for the assistance of the 'Supported Home' team, we are immediately looking for a Social Worker for 32 hours (89% employment).
.Tasks:
- Building and maintaining a network to help our clients, both inside and outside the organization .
- Finding out the request for help and helping to find an appropriate offer, in all areas of life and in cooperation with other parties .
- Finding solutions for complex requests for help, working in a planned way
- Providing support in arranging various practical matters
- Reporting in the digital client file
- Receives direction from and is accountable to the Assistance Program Coordinator
Functional Requirements:
- A relevant completed college education
- A minimum of two years of relevant work experience
- Affection with the target group of De Regenboog Groep
- Knowledge of the Amsterdam social map and at home in the network
- An open attitude, socially strong, motivating, able to distinguish and structure main and minor issues, connecting and inventive .
- Dare to dance outside the lines, do not depend on beaten tracks and fixed structures
- Able to work independently and in a team
